Appointment Scheduling
630 469 7700
All appointment scheduling is done
at a central phone center. Please make sure
that you clarify the office location for
which you wish to make your appointment.
Also, please remember to bring your most
current insurance card to each visit.
The first step in getting advice on howto care for your child is knowing how
to call your pediatrician. Before calling
our office, have a pencil and paper
ready so you can take notes on what
the nurse or doctor tells you.
In order to get the greatest benefit
from your call, please gather this
information before calling.
- What symptoms your
child is experiencing
- Time when symptoms started
- Past related history/conditions
- Any chronic illness your child may have
- Your child’s temperature and
how it was taken (rectal or oral)
- Your child’s approximate weight
- The names and dosages of any
medication your child is taking
- Pharmacy telephone number
Evening and Weekend Hours
We have limited evening and weekend
appointments available for acutely ill
children. We try to give priority to those
children who come home sick after school
or daycare and need to be seen the same day.
After Hours Calls
After regular office hours, we ask that
you only call us for emergencies or urgent
problems that can not wait until morning.
After hours calls will be received by an
answering service and then transferred to
the physician on call or the telephone nurse
who is covering that day. The doctor or
nurse will usually return your call within
15 minutes. If you do not receive a call
back within one hour in a non emergency
situation, please call again.
Emergency Calls
Day or Night
Call 911 for life-threatening emergencies
for which your child may need immediate
resuscitation–your child is not breathing,
is choking or is unconscious, for example.
Call our office at 630 469 7700 for minor
emergencies such as difficulty breathing,
wounds that need suturing or fractures.
When you call select the emergency
option and state clearly “THIS IS AN EMERGENCY”.
For poisoning, call the poison control
center at 800 942 5969.
Non - Medical Questions
If you are calling about the status of
a referral to a specialist, laboratory
results or a question about your child’s
immunization status, please expect the
receptionist to take your information
and then a nurse will call you back.
Sometimes there is a delay of 2-3 days,
until your child’s chart and needed
information can be accessed. If you
are requesting a copy of your child’s
medical record call 630 545 7635.
Cancellation Policy
We realize there are times when you
may need to reschedule or cancel an
appointment. We ask that you notify
us at least 48 hours before a well child
examination and as soon as possible before
an acute illness visit. Repeated occurrences
of non-compliance with our policy may
cause you to incur a $25.00 charge.
Coverage of the Practice
We encourage parents to meet several
of our pediatricians before choosing a
primary physician for their child. Our
physicians alternate office hours, days
off and holiday coverage. Therefore, your
child may be seen by a covering physician
in some cases. Your choice of physician
will be respected for all routine visits,
and whenever possible, for sickness and
emergencies. When follow-up care is
needed, please schedule the appointment
with the same physician, as he or she would
be most familiar with your child’s illness.
This may occasionally necessitate a visit
to an office out of your service area.
As a group, we agree on all points in the
important areas of handling the diseases
of children. Furthermore, we will make
every effort to give the same advice in
basically unimportant or debatable areas
of health care. In spite of our efforts for
consistency in advice, slight variations may
occur in the advice given. If this should
occur, please talk to us for clarification.
Sick Children Visits
If you wish to have a sick child examined,
simply ask for an appointment and state
which physician or office you prefer.
The receptionist will offer you the
first appointment time available.
Most sick children can be brought to
the physician’s office. Fever is not a
contraindication for bringing a child
to the office for examination. Please
advise the receptionist if your child
has a contagious disease or a rash.
You will be instructed to use
a special entrance.
Have your child wear clothing that
is easy to remove for examination.
To be certain we can give your child
the attention he/she deserves, always call
first before going to any of our offices.

Well Child Examinations
We recommend that you schedule well
child check ups with one or two of the
same physicians, in order to help the doctor
get to know and better take care of your
child. Please call two to three weeks in
advance to schedule routine appointments.
We do school, camp and sport examinations
year-round. At this time, school and sport
examinations are valid if done within
the year of the required date. For your
convenience, try to schedule school exams
in early spring and summer in order to
avoid the late summer rush. At that time,
it may be difficult to schedule a requested
time or physician of your choice. For
baby check-ups or follow-up visits for sick
children, make your next appointment as
you leave the office, when they’re first seen.
Please call us between 9:00 a.m., and
4:00 p.m. on weekdays at 630 469 7700,
to make these appointments.
Before Your Child's Examination
Explaining or describing to your child
what happens during an office visit with
the physician is recommended prior to your
appointment. It is advisable not to make
promises to a child that you may be unable
to keep; such as, “you will not get a shot,”
or “the doctor will not use a stick to look
at your throat.” Pre-examining or looking
your child over before an office visit is a
good idea. This procedure educates the
younger child on what to expect during
the office visit. A better, more satisfactory
exam can be performed on a cooperative,
understanding child. Furthermore,
parents can gain useful information,
have a more informed basis for the need
for an examination in the case of a sick
child and be prepared for more relevant
questions and answers for the physician.
Over time, this policy will enhance the
child’s awareness and appreciation for the
components of good health care, prevention
and medical self-help. This achievement
would probably be the single most
important goal of our pediatric care.
For your child’s protection, and to aid
in diagnosis, do not give gum, candy
or food to the child before the exam.
Objects in the mouth may cause your
child to choke during the examination
or when an injection is given. Some
candy can discolor the mouth and throat,
making it more difficult or impossible
to make an accurate evaluation. Have
your child undressed for the examination
and also have a coat or a blanket to
use while waiting.
Please do not bring food for your
child to eat while in the office!
After Your Child's Examination
Follow the directions given to you by
the physician and nurse. If you do not
understand their instructions or have any
questions, please ask for clarification. Give
medication as directed on the prescription.
Call us if your child develops any new
symptoms. It usually takes two to three
days before your child will improve after
antibiotics have been started. Fever may
continue and in some cases even rise, but
unless associated with other new symptoms,
should not be a cause for undue concern.
Viral infections such as colds and flu do
not respond to antibiotic therapy. If we
have not prescribed antibiotics for your
child because he/she has a viral infection,
observe closely for any newly developing
symptoms such as rash, nasal discharge,
swollen glands, stiff neck, difficulty in
breathing or trouble urinating. There may
be times when several days after being
examined for an illness, your child will
develop a secondary infection (e.g. ear
infection or bronchitis). This new
development may necessitate
another office visit.
Sometimes the necessity for a second visit
during an illness may be disturbing to
parents since they may perceive that the
problem should have been resolved with
the first exam. It is important for parents
to realize that this will occasionally occur,
it is not uncommon in the pediatric age
group, and is frequently unavoidable.
We try to anticipate what the course of
your child’s illness may be and try to give
general advice for that illness, but naturally
we cannot predict in every individual case.
Some of the common problems to be
aware of are as follows:
Some temporary hearing loss may
be associated with an ear infection.
A tracheal cough can be very persistent
and resistant to treatment.
Bronchitis and bronchiolitis usually
last at least 10-14 days.
It may take 7-10 days after an illness
before a child’s appetite returns to normal.
This is a normal part of convalescence.
Urinary tract infections should be
followed carefully until resolved. It is
important to observe your child for
recurrence of this infection.
Most viral rash conditions are no longer
contagious after the rash starts to fade.
Medical Advice
If you need further medical advice
about your child’s illness or well
being after reading this booklet,
you may call the office:
Monday through Friday
7:00 a.m. - 8:30 p.m.
Saturday and Sunday
8:00 a.m. - 5:00 p.m.
A nurse who has been specially trained
to answer most questions screens (triages)
all medical advice calls. If the nurse
cannot help you, she may ask that
you schedule an appointment or
will have a physician call you back.
During busy phone times, the receptionist
will take your message and have the nurse
call you back. While waiting for a call-back,
try to keep your line open. In general, we
try to return calls within one hour. If your
call has not been returned within 4 hours,
please call back. Keep in mind that
Monday mornings are the busiest.
School Forms
We ask that you bring any school, day-care
and/or camp forms to your check-up.
Please fill out your portion of the forms as
thoroughly as possible, BEFORE the visit.
If you need a form or immunization record
filled out at another time, please allow one
week for completion of these forms.
Other Services Offered
We are here to assist you in every way possible. Your pediatrician, working with
you, can provide the best health care available for your child because of his/her
deep interest in pediatrics. Special training in the physical and emotional development
of children and childhood illnesses makes your pediatrician uniquely qualified
to care for your child through age 18 years, or longer if desired.
A great deal of attention is paid to preventive health care, including
a planned immunization program, nutrition and visual screening.
Services include neonatology, infants, annual and school exams, camp
and sports physicals, in addition to care for the sick child and referral
to other departments in our clinic or elsewhere.
Care is always available during scheduled office hours for injuries and accidents,
including suturing of lacerations and fracture care. X-ray and laboratory services
are available for non-urgent procedures such as wart removal and other minor
surgical and non-surgical procedures. These include the management of nail
infections and administering IV fluids. Emergency care after regular office hours
is arranged specifically with the on-call doctor.
Should your child need special diagnosis and treatment, we offer various
sub-specialties such as Allergy, Cardiology, Dermatology, Dietary, Gastroenterology,
Ophthalmology, Orthopedics, Otolaryngology (ENT), Physical Therapy, Podiatry,
Surgery and Urology within the Clinic.
We also have a close working relationship with many Pediatric Subspecialists
in the area to whom we can refer if your child’s needs so dictate.
Consultations are available for school adjustment problems, asthma,
learning disability, nocturnal enuresis (bed-wetting), bowel problems,
discipline, behavior and adolescent problems by special appointment.
Medication Refills
Please call between 9:00 a.m. and 3:00 p.m.
Monday through Friday for prescription
refills. Be prepared to give the prescription
number, name of medication and phone
number of the pharmacy to the receptionist.
Also, plan ahead so you do not run out
of important medications.
Managed Care
If you belong to one of the Managed Care
(HMO, PPO, POS, etc.) medical plans,
there are certain important points you should
be aware of to maximize your coverage.
It is essential that you read your contract,
including all the fine print, so that you are
aware of services covered and not covered
by your plan. Each plan is different in this
regard and misunderstandings can be
avoided if you are familiar with your
specific plan, and its requirements prior
to seeking care. Some of the Managed
Care plans require a co-payment.
This fee is due at time of service.
Office Fees
A schedule of fees for regular, night,
Saturday, Sunday, holiday, emergency
and after hour visits is available on request.
Please check your service slip before
leaving. If you have any questions, bring
them to our attention at this time.
Follow Up Fees
When the physician requests that a
follow-up visit be made for your child,
this will be noted on the chart and a
follow-up visit fee will usually be charged.
We try to minimize follow-up visits, but
obviously they are important to your child’s
health care. If for some reason you do not
feel that a follow-up visit is necessary,
please call, so that it can be discussed
and the physician be made aware.
Insurance Payments
All billing and insurance questions
should be directed to the Customer Service
Department at 630 942 798, Monday
through Friday during business hours.
They will assist you promptly and help
you obtain the benefits you are entitled to.
Payments
Charges for medical services are due
and payable at the time of service. At the
conclusion of your visit, an itemized bill
showing services provided and the cost for
each service will be presented to you for
payment. Payment may be made by cash,
check or major credit card. If you choose
to have our insurance department file
electronic or automatic claims, you will be
responsible for 100% of charges at the time
of service for all co-payments, non-covered
services and your deductible. After your
deductible is met, 20% of the total charge
is required at the time of service. Any
remaining balance after your insurance
company has paid will be your responsibility.
We do not become involved in disputes
between you and your insurance company
regarding usual and customary charges,
deductibles or covered charges other than
to supply requested factual information.
